Description (Service)	Team of mental health professionals who provide specialized services to persons with severe and persistent mental disorders assessment and symptom management pharmacotherapy individual therapy integrated treatment of mental health and substance use problems housing support assistance with daily living (medication management, budgeting, grocery shopping, etc.) educational support employment support socialization and recreation skill development crisis intervention advocacy family and social support and education partnership with other resources
Hours	Mon-Fri 8 am-4 pm
Eligibility	French-speaking individuals 18 years of age and older who live in the Ottawa area and have been diagnosed with schizophrenia, mood disorders or other psychotic disorders (e.g. schizoaffective disorder) * Services are also provided to individuals who are experiencing significant difficulties in housing, social, employment or educational functioning
Languages	French
Application	By referral through the Central Intake Committee * Applications are then forwarded to the appropriate ACT team, which will assess the individual to determine eligibility
