Description (Service)	Team of mental health professionals who provide specialized services to persons with severe and persistent mental disorders accompanying services as well as support during the person's daily life Services: symptom evaluation and control pharmacotherapy individual therapy integrated mental health and drug abuse treatment home support assistance in organizing daily life educational support employment support socialization and leisure activities ability development crisis intervention
Hours	Mon-Fri 8 am-4 pm
Eligibility	Persons 18 years and older with chronic and/or persistent mental illness persons who have frequent or prolonged admissions to hospitals for mental health reasons Persons who have a significant and apparent functional disorder that is defined as: the inability to keep up with daily activities, to ensure personal safety and to participate in existing services
