Description (Service) Assertive Community Treatment Team (ACTT): Voluntary program with a trans-disciplinary team which may include:
  • psychiatrist
  • nurses
  • social workers
  • mental health workers
  • vocational rehabilitation worker
  • recreation therapist
  • occupational therapist
  • addictions worker
  • peer support worker
  • provides a wide range of comprehensive community-based services to clients on a year-round basis
  • client-centered approach
  • family and significant other involvement with client consent
  • multidisciplinary treatment team
  • illness treatment
  • improved quality of life goals
Hours
  • Service Mon-Sun 24 hours
  • Administration Mon-Fri 8:30 am-4:30 pm
  • ACTT Program Sat-Sun 10 am-6 pm
Eligibility
  • persons between the ages of 16 and 75 with persistent or chronic mental illness
  • service is intended for those with the highest intensity of need due to complex, long-term, serious psychiatric issues
  • eligibility will be determined by a consultation with the client, ACTT staff, and a central intake committee
  • individuals who think they might be eligible for service, or are interested in obtaining it, are encouraged to discuss it with their current service providers
