| Eligibility | Youth aged 16-25 and Adults 25 years and older with a severe and persistent mental illness in Intensive Case Management program * SAPACCY youth ages 12-29 yrs |

| Application | Community referrals from mental health professionals, doctors, social workers, self or family for mental health case management * individuals should apply through the Mental Health Community Support Services (MHCSS) Central Intake portal housed at the Canadian Mental Health Association - Ottawa Branch at 613-737-7791 SAPACCY referrals, self/family/other see website to book appt or call Psychiatric Assessment clinic referrals must come through a recognized partner agency such as a Community Health Centre, Mental Health Community Support Services or Ottawa Youth Justice Service Network partner agency |
