Description (Service)	Long-term support for individuals living with severe and persistent mental illness mental health case management women's group 1st connection monthly luncheon back to school back to work program psychiatric assessment client Substance Abuse Program for African Canadian and Caribbean Youth (SAPACCY): culturally adaptive Mental Health and Addiction support
Hours	Mon-Fri 8:30 am-4:30 pm
Eligibility	Youth aged 16-25 and Adults 25 years and older with a severe and persistent mental illness in Intensive Case Management program * SAPACCY youth ages 12-29 yrs
Languages	English, Farsi, French, Polish
Application	Community referrals from mental health professionals, doctors, social workers, self or family for mental health case management * individuals should apply through the Mental Health Community Support Services (MHCSS) Central Intake portal housed at the Canadian Mental Health Association - Ottawa Branch at 613-737-7791 SAPACCY referrals, self/family/other see website to book appt or call Psychiatric Assessment clinic referrals must come through a recognized partner agency such as a Community Health Centre, Mental Health Community Support Services or Ottawa Youth Justice Service Network partner agency
