Application Community referrals from mental health professionals, doctors, social workers, self or family for mental health case management * individuals should apply through the Mental Health Community Support Services (MHCSS) Central Intake portal housed at the Canadian Mental Health Association - Ottawa Branch at 613-737-7791SAPACCY referrals, self/family/other see website to book appt or call Psychiatric Assessment clinic referrals must come through a recognized partner agency such as a Community Health Centre, Mental Health Community Support Services or Ottawa Youth Justice Service Network partner agency
