Description and Services
Description (Service) Mental health treatment, rehabilitation and support services for identified clients with severe and persistent mental illness

Services:
  • crisis intervention (clients of ACT only)
  • education, support and consultation to clients' families
  • medication prescription, administration, monitoring
  • social, interpersonal relationship, and leisure-time skill training
  • substance abuse counselling
  • support with activities of daily living
  • symptom assessment, management, individual supportive therapy
  • work related services
Hours Mon-Fri 8:30 am-4:30 pm, Sat-Sun 10 am-6 pm * after-hours and crisis intervention for clients within the program Mon-Sun 24 hours
Eligibility Persons 16 years old and over with persistent or chronic mental illness

Persons with a high service use as defined by being in an institution on the basis of a mental illness for more than 90 days continuous, 50 days in the past year or 150 days over the past 3 years. Additional consideration is given to clients who exhibit difficulty with homelessness and incarceration

Persons with a significant functional impairment demonstrated by: the inability to maintain activities of daily living, the inability to maintain personal safety, and the inability to participate in existing services

Complexity of presenting problems given priority, such as coexisting substance abuse of more than 6 months, co-morbidity of psychiatric diagnosis, or criminal justice involvement
